      Дивертикул Меккеля (ДМ) относится к врожденной патологии развития желудочно-кишечного тракта. В статье рассматриваются вопросы эмбриологии и особенности строения дивертикула Меккеля, а именно локализация данного образования и встречающуюся в нем гетеротопическая ткань, а также описаны возможные клинические проявления, такие как желудочно-кишечное кровотечение и диверикулит, и подходы к диагностике и лечению.
 
      Ключевые слова: дивертикул Меккеля; морфология; гетеротопическая ткань; половые особенности; возрастные особенности; желудочно-кишечное кровотечение.

Pimenov I.A. Meckel's Diverticulum: A Brief Up-To-Date Literature Review

        Preoperative diagnosis of complicated Meckel’s diverticulum (DM) is complicated due to the clinically and visually overlapping features of other acute surgical, inflammatory diseases of the abdominal cavity (to date, less than 10% of symptomatic DM is diagnosed). This makes it important to monitor and periodically summarize the results presented in up-to-date research in this area.

       The subject matter of this brief review is DM and congenital pathology of the gastrointestinal tract. In my paper, I present data of domestic and foreign articles of 2010—2018 on issues of embryology, structural features of DM, possible clinical manifestations, such as gastrointestinal bleeding, diverticulitis, intestinal obstruction, hernia Littre and others, as well as recommended approaches to treatment and diagnosis of this pathology.

For my purposes I used problem and source analysis of more than 40 sources on the forms, sizes, location of DM in the abdominal cavity, and morphological features of its mucosa.

         Meckel's diverticulum is the most common congenital anomaly of the gastrointestinal tract. DM is usually detected randomly with laparotomic, laparoscopic surgical procedures performed in threatening conditions due to difficulty in detecting and differentiating by symptom complex from other abdominal pathological processes, such as appendicitis and acute pancreatitis, due to the absence of specific symptoms in patients with complications of this pathology. Researchers most often detected in children under the age of 10 years. DM is located at a distance of 20—100 cm from the ileocecal valve, the average length is from 2 to 8 cm. In most cases (73.3%) DM has a free tip, but it is attached to the anterior abdominal wall or mesentery root. The predominant form is conical (54%), but also cylindrical and pear-shaped. The width of the base varies from 0.5 cm to 5 cm. Uncomplicated DM occurs equally in men and women, and complicated one prevails in men 3—4 times more often than in women. The frequency of malignant transformation of DM varies from 1 to 6.4%. One of the leading causes in the development of complications in patients with Meckel's diverticulum is the presence in 50—60% of cases of heterotopic tissue, more often it is the mucous membrane of the stomach (60—85%) and pancreatic tissue (5—6%); researchers describe other cases (mucous skin jejunum, duodenal or large intestine, hepatobiliary tissue and endometrium) as occurring less frequently. The mucosa may ulcerate and cause inflammation, peritonitis, and intestinal bleeding. The most accurate method for the diagnosis of Meckel's diverticulum is the radioisotope method with the introduction of the technetium Tc99m isotope, which detects the ectopic gastric mucosa in DM.

I conclude that adequate knowledge of the embryological, clinical, pathological, and radiographic characteristics of Meckel's diverticulum will facilitate early and accurate diagnosis of complex cases.
